The TyG index converts two routine fasting labs into a practical surrogate of insulin resistance, enabling cardiometabolic risk screening without a specialized insulin assay.
A 48-year-old man with metabolic syndrome has fasting triglycerides 150 mg/dL and fasting glucose 90 mg/dL.
Inputs:
TyG index = ln((150 × 90) ÷ 2) = ln(6750) ≈ 8.82 — descriptive; compare with local population cut-points.
The constants were chosen empirically so that the index correlates with euglycemic-hyperinsulinemic clamp-derived insulin sensitivity in the derivation cohort.
TyG requires no insulin assay and correlates with HOMA-IR and clamp measures. Both are research-oriented surrogates; neither alone should drive clinical decisions.
