Assesses metabolic syndrome using the updated NCEP ATP III criteria, which require at least 3 of 5 components: elevated waist circumference, elevated triglycerides (or treatment), reduced HDL (or treatment), elevated blood pressure (or treatment), and elevated fasting glucose (or treatment).
Metabolic syndrome = ≥ 3 of 5 ATP III criteria (waist, triglycerides, HDL, blood pressure, fasting glucose, each with treatment exemption)
Present when ≥ 3 of 5 criteria are met (ATP III)
The updated NCEP ATP III criteria (2005) define metabolic syndrome as the presence of at least three of five components: abdominal obesity (waist ≥ 102 cm men / ≥ 88 cm women, or BMI ≥ 30), triglycerides ≥ 150 mg/dL or drug treatment, HDL < 40 mg/dL men / < 50 mg/dL women or drug treatment, blood pressure ≥ 130/85 mmHg or drug treatment, and fasting glucose ≥ 100 mg/dL or drug treatment.

Assesses metabolic syndrome using the updated NCEP ATP III criteria, requiring at least 3 of 5 components: elevated waist circumference, elevated triglycerides (or treatment), reduced HDL (or treatment), elevated blood pressure (or treatment), and elevated fasting glucose (or treatment).
Metabolic syndrome is present when at least 3 of 5 ATP III criteria are met: waist ≥ 102 cm (men) or ≥ 88 cm (women); triglycerides ≥ 150 mg/dL or drug treatment; HDL < 40 mg/dL (men) or < 50 mg/dL (women) or drug treatment; blood pressure ≥ 130/85 mmHg or drug treatment; fasting glucose ≥ 100 mg/dL or drug treatment. This calculator uses the classic 102/88 cm waist cutoffs.
Metabolic syndrome identifies patients with clustered cardiometabolic risk factors who warrant aggressive lifestyle modification and screening for diabetes, hypertension, and cardiovascular disease.
A 50-year-old woman has a waist of 90 cm, triglycerides 160 mg/dL, HDL 45 mg/dL, blood pressure 135/85 mmHg, and fasting glucose 110 mg/dL, on no relevant drug treatment.
Inputs:
All 5 ATP III criteria are met (waist, triglycerides, HDL, blood pressure, and fasting glucose), so metabolic syndrome is present.
Any 3 of the 5 ATP III criteria (waist, triglycerides, HDL, blood pressure, glucose), with patients on drug treatment for any criterion counted as meeting that criterion.
The classic ATP III cutoffs are 102 cm (men) and 88 cm (women), or BMI ≥ 30 kg/m². Some population-specific guidelines use lower cutoffs (e.g., 90/80 cm for Asian populations).
